Recurring Donations

INSTRUCTIONS:
In box No.1 enter the amount of your donation or pledge.
In box 2 enter the number of times you wish to donate in any one time frame. ( example: 1 time per month or 3 times per year)
In box 3 enter how often you wish to donate.
In box 4 enter how long you wish the donation/partnership to continue. (Anywhere from a single 1 time donation to a choice of Until I Decide to Cancel)
